Statement and Official Nomination

I Certify that the statements made on this Nomination Form are true and correct to the best of my belief.

If accepted as a participant, I agree to follow the course of the program and abide by the following OCSC regulations:

• Participants should participate in the educational program to the best of their abilities
• As the course contents and schedule are fixed, no change may be requested by participants
• Participants must return to their home country upon completion of the education program and resume work in their country
• Participants should not extend the length of the training course or extend their stay for personal convenience
• Participants should not be accompanied by any family members
• Each participant will be sharing the hotel room with another participant of the same gender throughout the program
• Participants are to assume responsibility for any personal expenses incurred, regardless of implementation of the course
• Participants are not permitted to change the flight schedule arranged by OCSC for personal convenience
